Driver Hiring Requirements

  • Must hold a valid driver’s license

  • Must have active vehicle registration and insurance

  • Must pass background and drug screening

  • Must submit all required onboarding documents

Incomplete submissions will delay processing.

What You’ll Do

• Transport clients to medical and essential appointments

• Ensure safe, on-time, professional service

• Follow assigned routes and schedules

• Assist passengers as needed (non-medical)

• Communicate with dispatch

Required Onboarding Documents (Once Hired)

  1. Driver’s License (Front and Back)

  2. Vehicle Registration
    (All vehicles to be used)

  3. Insurance Declaration Page
    (Declaration page only – no ID cards)

  4. Motor Vehicle Record (MVR)
    (Obtained from the DMV website)

  5. Signed Background Check Form

  6. Signed Recruitment & Duty Form

  7. Drug & Random Drug Test Consent Form

  8. W-9 Form

  9. ACH Direct Deposit Form

Driver Requirements

• Must be 18 years or older

• Valid driver’s license

• Reliable, insured, and registered vehicle

• Clean driving record

• Ability to pass background and drug screening

• Smartphone with GPS
