To apply for a driver position with Medi Rides & Transport Services LLC, please upload the required documents and images using the form. Ensure that your files meet the following guidelines:

Accepted File Types:

  • Images: .png, .jpg, .tiff, .heic

  • Documents: .docx, .doc, .pdf, .txt

Instructions:

  1. Complete all required form fields, including your Full Name and Email Address.

  2. Upload your documents and images using the Upload section on the right. You may upload multiple files if necessary.

  3. Double-check that each file is clear, legible, and in the accepted format.

  4. Click Submit only once after all files are uploaded. You will receive a confirmation that your submission was successful.

Important Notes:

  • All uploaded files must be related to your driver application.

  • Maximum file size per upload is 20MB.

  • By submitting your files, you confirm that all information provided is accurate.
